A sixth person is dead after a fatal two vehicle collision in Mapleton, Ont. last week, said Ontario Provincial Police.

A passenger van and sports utility vehicle collided around 7:30 p.m. on June 12 in the Township of Mapleton, located about 35 kilometres northwest of Kitchener-Waterloo, said an OPP news release Saturday.

They said five children, aged four, six, eight, 10 and 12 years old, were pronounced dead after the crash and five others in the passenger vehicle sustained injuries.

The driver and lone occupant of the SUV, a 42-year-old man from Brampton, died in hospital on Friday from his injuries, said the OPP. Initially, police said the man died on Tuesday, but later corrected the date to Friday.

A funeral service will be held on Saturday afternoon for the five children of the Doerksen family. Peter, 12, Helena, 10, Sara, 8, Maggie, 6, and Susy, 4, will be remembered by friends and family at a rural church outside the town of Elmira.

An investigation into the incident is ongoing, with police urging anyone with information or dashcam footage to reach out or contact Crime Stoppers.
